Waiver, Release and Hold Harmless Agreement
As consideration for being allowed to attend The Sponsor Luncheon sponsored by the Suffolk County Women’s Business Enterprise Coalition (“SCWBEC”) on June 24, 2021 at The Meadows Club (the “Event”), the undersigned, on her (or his) own behalf, (“I” “Myself” or ”Me”), acknowledge, understand, and agree to the following:
I am at least 18 years old and am legally competent to understand and complete this Agreement.
I agree to comply with all current U.S. Centers for Disease Control and Prevention guidance, state and local laws, rules, regulations and health codes, including any applicable business requirements of the Event venue,for reducing the spread of COVID-19. I acknowledge that on the day of the Event I may be asked to provide proof that I have been “fully vaccinated” against COVID-19; or provide proof of a negative a COVID-19 lab test I had performed within 72 hours prior to the Event; and/or wear a mask while attending the Event and/or socially distancing from others (collectively the “Requirements”). An individual is considered “fully vaccinated” for COVID-19 ≥2 weeks after receiving the second dose in a 2-dose series or ≥2 weeks after receiving a single-dose vaccine. Given the evolving situation with respect to Covid-19, all applicable Requirements will be communicated by SCWBEC to the email address provided during the registration process in advance of the Event.
I acknowledge that I may be denied access to or forced to vacate the Event if I do not comply with the Requirements.
I certify that I have adequate insurance to cover any injury, sickness, illness, or damage that I may suffer while present at the Event, or if not, that I shall bear all costs and expenses associated with or arising out of any injury, sickness, illness I may suffer while at the Event.
I understand that photographs or videos may be taken at the Event. These photographs or videos may be used by SCWBEC in print publications, online publications, presentations, websites, and in social media for educational or promotional purposes. I consent for my photo, image or likeness of me to be used in any publication, presentations, websites or on social media, for any lawful purpose whatsoever, without payment or reimbursement to Me
I hereby release and agree to hold SCWBEC , its officers, directors, agents, and/or employees, (“RELEASEES”) harmless from, and waive on behalf of Myself, my heirs, and any personal representatives, any and all causes of action claims, demands, damages, costs, expenses and compensation for damage or loss to Myself and/or property that may be caused by any act, or failure to act of Releasees, to the fullest extent permitted by law. I understand that this release discharges Releasees from any liability or claim that I, my heirs, or any personal representatives may have against the Releasees with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection with Me attending the Event.
I HAVE READ THIS WAIVER, RELEASE AND HOLD HARMLESS AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY AGREEING TO IT, AND AGREE TO IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
